I take the privacy rights of my patients very seriously. This Privacy Policy explains how I handle your personal information. Personal information is information that could identify you or relates to your identity.
What Personal Information Do I Collect?
To provide treatment, I will ask you to provide certain personal information. This may include:

Gender
Date of birth
Home address
Email address
Telephone number
Medical history
Over time, I will add details about the conditions for which you have consulted me, along with any remedies and/or other therapies I have prescribed or recommended.
How Do I Collect Your Personal Information?
All information is collected directly from you, usually at the time of your initial consultation.
The information is collected through a medical history form, which I will ask you to complete before or during your initial consultation.
When you provide your personal information, I will also ask for your consent to store and use your data. Your consent is required to support my compliance with data protection legislation.
I will subsequently add details of any consultations you have with me to your patient record.
How Do I Use Your Personal Information?
I use your personal information to:
Assess and analyse the conditions for which you have consulted me.
Prescribe or recommend appropriate remedies and/or other therapies.
Communicate with you regarding your consultations and treatment.
I may communicate with you by email, other digital methods, telephone or post.
Who Do I Share Your Personal Information With?
I do not share your personal information unless I have a statutory or legal duty to do so.
How Long Do I Keep Your Personal Information?
I need to retain your information for as long as you continue to consult me.
As patients may return for further consultations after a period of absence, I will retain your information for a minimum of seven years after your last consultation.
After this period, your paper file will be securely destroyed and any digital information will be erased from my computer systems.
Updating or Correcting Your Information
To help ensure that the information I hold is accurate and up to date, please inform me of any changes or corrections that need to be made.

Under data protection legislation, you have the right to request access to the personal information I hold about you.
You can make a request by contacting me, and I will endeavour to respond within 14 working days.
How Do I Store Your Personal Information?
My patient files are paper-based and are stored securely within my consulting room.
I take reasonable steps to protect your personal information against:
Loss or theft
Unauthorised access
Unauthorised disclosure
Unauthorised copying
Improper use
Unauthorised modification
Your email address, where provided, is stored securely on the servers of my email providers, which are currently BT Internet and Google Mail.

If you have a concern or complaint regarding my data protection procedures, please:
Contact me in writing using the contact details below and explain the nature of your concern or complaint.
I will acknowledge receipt of your concern or complaint within 30 days.
Your concern or complaint will be thoroughly investigated and responded to within a reasonable timeframe.
You will be kept informed of progress, and an outcome will be provided as quickly as reasonably possible.
Changes to This Policy
This policy may be updated from time to time.
If I make any material changes, I will make you aware of them.
